BE IT RESOLVED BY THE COUNCIL OF THE LEXINGTON-FAYETTE URBAN COUNTY GOVERNMENT:
Section 1 - That the Mayor, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized and directed to execute a Statement of Work (SOW) and a multi-year Services Agreement, which are attached hereto and incorporated herein by reference, with Metaformers, Inc., for managed data migration and hosting services in the Cloud.
Section 2 - That the Chief Information Officer, or her designee, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized to execute any necessary documents related to the procurement.
Section 3 - That an amount, not to exceed the sum of $208,508.00, be and hereby is approved for payment to Metaformers, Inc., from the following accounts: #2604-210101-71208 ($6,339.25), #2607-210101-71208 ($5,327.84), #1105-210101-71208 ($63,332.91), and #1101-202505-76102 ($133,508.00), pursuant to the terms of the SOW and Services Agreement.
Section 4 - That this Resolution shall become effective on the date of its passage.
