BE IT RESOLVED BY THE COUNCIL OF THE LEXINGTON-FAYETTE URBAN COUNTY GOVERNMENT:
Section 1 - That the Mayor, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized to execute a renewal Agreement with Anthem Health Plans of Kentucky, Inc., for administrative services and stop loss insurance coverage related to Health Insurance Plans PPO 1, PPO 2, HSA 1 AND HSA 2, for a one-year term beginning January 1, 2015, for the Urban County Government.
Section 2 - That this Resolution shall become effective on the date of its passage.
