Summary
Authorization to approve Amendment #2 to the contract between the LFUCG and Corizon, Inc. This amendment will provide funding for a full time ACA Coordinator to assist with filing of medical claims, and to increase the contract by the mandated CPI increase. The additional cost is $49,812 for the ACA coordinator and $59,556.46 for the CPI increase for a total increase of $109,368.46. The FY16 base contract price will be $271,708.68 per month, an increase of $9,114.04 per month. The request is fully funded in the proposed FY16 budget and execution will be contingent upon funding.
