BE IT RESOLVED BY THE COUNCIL OF THE LEXINGTON-FAYETTE URBAN COUNTY GOVERNMENT:
Section 1 - That the Mayor, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized and directed to execute the First Amendment to the Agreement with Access Wellness Group, Inc., which is attached hereto and incorporated herein by reference, extending the Agreement to provide Employee Assistance Program (EAP) services through December 31, 2021, with five subsequent one year extensions, at a cost not to exceed $56,700.00 in FY 2021.
Section 2 - That an amount, not to exceed $56,700.00 in FY 2021, be and hereby is approved for payment to Access Wellness Group, Inc. from account 1101-160504-1924-71204, pursuant to the terms of the Agreement, with funding for future fiscal years contingent upon the appropriation of sufficient funds.
