Summary
Authorization to Execute Change Order No. 2 for $9,837.88 with Churchill McGee, LLC, Lexington, Kentucky for the Public Safety Operations Center Phase 1B Renovation construction project Contract #287-2014. The new contract total is $6,136,871.67. A total of seven (7) working days will be added to the project schedule. Per Proposal Request 5, reroute the interior underground storm drain piping in order to conceal the riser in Area B that would not fit into a standard wall for a cost of $2,216.21. Per Proposal Request 9, add seven (7) working days to the construction schedule due to adverse weather days. Per Proposal Request 13, the existing concrete slab was found to not be adequately thick for laying new CMU on top of it, so a portion of the slab as described in the attached documents in rooms Break Out 125 and 126 shall be replaced with a new 4" slab for a cost of $5,381.47. Per Proposal Request 15, a new concrete slab is to be placed on an area of unsuitable soils, which needs to be taken out, disposed of, and replaced with DGA before new concrete is placed for a cost of $2,240.20.
