This is a request to execute an agreement with HealthFirst Bluegrass, Inc. to provide part-time pediatric services at the Samuel Brown Health Center at a cost of $174,219 with $50,000.00 due upon signing of the contract. The remainder will be paid in installments of $10,351.00 on the 30th of each month during the contract period. The contract is for one year beginning February 1, 2015. Funds are budgeted.

BE IT RESOLVED BY THE COUNCIL OF THE LEXINGTON-FAYETTE URBAN COUNTY GOVERNMENT:
Section 1 - That the Mayor, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized and directed to execute the Professional Services Agreement, which is attached hereto and incorporated herein by reference, with HealthFirst Bluegrass, Inc., for the provision of part-time pediatric services at the Samuel Brown Health Center.
Section 2 - That an amount, not to exceed the sum of $174,219.00 be and hereby is approved for payment to HealthFirst Bluegrass, Inc., from account #6002-160504-71203, pursuant to the terms of the Agreement.
Section 3 - That this Resolution shall become effective on the date of its passage.
