BE IT RESOLVED BY THE COUNCIL OF THE LEXINGTON-FAYETTE URBAN COUNTY GOVERNMENT:
Section 1 - That the Mayor, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized and directed to execute the Second Amendment and Extension to the existing Health Services Agreement, which is attached hereto and incorporated herein by reference, with CHS TX, Inc. dba YesCare, successor by merger to the rights and obligation of Corizon, LLC, for the Division of Community Corrections, increasing the Contract price by the sum of $417,076.00, from $5,151,303.00 to $5,568.379.00 FY 23.
Section 2 - That an amount, not to exceed the sum of $5,568.379.00 in FY 2023, be and hereby is approved for payment to CHS TX, Inc. dba YesCare, successor by merger to the rights and obligation of Corizon, LLC, from account #1101-505402-5421-71203, pursuant to the terms of the Agreement.
