BE IT RESOLVED BY THE COUNCIL OF THE LEXINGTON-FAYETTE URBAN COUNTY GOVERNMENT:
Section 1 - That the Mayor, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized to execute the Agreement, which is attached hereto and incorporated herein by reference, with Meijer Great Lakes Limited Partnership, d/b/a Meijer Pharmacy Division, to supply the influenza vaccine and administer influenza vaccinations to eligible Lexington-Fayette Urban County Government employees, at a cost of fifteen dollars ($15.00) per employee, during the 2016 - 2017 flu season.
Section 2 - That an amount, not to exceed the sum of $15.00 per employee, is hereby approved for payment to Meijer Great Lakes Limited, d/b/a Meijer Pharmacy Division, from Account No. 1101-160504-71203, pursuant to the terms of the Agreement.
Section 3 - That this Resolution shall become effective on the date of its passage.
