BE IT RESOLVED BY THE COUNCIL OF THE LEXINGTON-FAYETTE URBAN COUNTY GOVERNMENT:
Section 1 - That the Mayor, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized and directed to execute an Occupational Wellness Administrator and Services Agreement with Occupational Health Centers of the Southwest, P.A., d/b/a Concentra, for occupational health services, for a one (1) year period beginning on November 1, 2018.
Section 2 - That this Resolution shall become effective upon passage of Council.
