BE IT RESOLVED BY THE COUNCIL OF THE LEXINGTON-FAYETTE URBAN COUNTY GOVERNMENT:
Section 1 - That the Mayor, on behalf of the Lexington-Fayette Urban County Government, be and hereby is authorized and directed to execute an Addendum to the Health Services Agreement, which is attached hereto and incorporated herein by reference, with Marathon Health, LLC, for certain health care services offered at the Samuel Brown Health Center; extending the Agreement through January 18, 2019.
Section 2 - That an amount, not to exceed the sum of $1,697,510.00 be and hereby is approved for payment to Marathon Health, LLC, from account #6002-160504-71203, pursuant to the terms of the Agreement as amended.
Section 3 - That this Resolution shall become effective on the date of its passage.
